CHSCPR Application Form: Executive Committee Positions
The Chicago Healthcare System Coalition for Preparedness and Response (CHSCPR) is a collaboration of healthcare organizations and providers, the public health department, and community partners working together to care for our community before, during, and after an emergency. CHSCPR is an inclusive body open to all organizations/entities that provide or support healthcare services within the City of Chicago and that wish to work collaboratively on emergency preparedness, mitigation, response, and recovery activities.
CHSCPR’s Executive Committee meets virtually the second Tuesday of every month. In order to be nominated for an Executive Committee position, all applicants must meet the following eligibility requirements:

Member

Active engagement as demonstrated by attendance at 75% of the prior calendar year’s All Partner Coalition meetings and committee meetings

Expresses a willingness to serve in the elected position July 1, 2024 until June 30, 2026

Receives organizational support for participation

Provides a resume/CV/bio for distribution to the membership
